ACHA Men's Division 2 (M2) Documents	Click here for USA Hockey information for the 2010-2011 season. -ACHA League Info Document Required for all ACHA local leagues -D2 Checklist This will answer some startup questions and guide you along -2010-11 ACHA Team Application Divisional declaration for 2009-2010 season -2010-2011 USA Hockey Registration Required to be submitted before first game is played and full web access is granted -D2 Info Request This document helps identify new team staff officials -ACHA Player Agreement Forms Required by all players to be completed and submitted to Commissioner by Oct 31st -ACHA Game Contract Should be used for all games with other ACHA teams -ACHA Incident Report Use this when reporting a game incident, notify ref this is being completed.. Either ACHA team can submit this form if Commissioners notice is required to review this game or further supplimenatry discipline or extenuating event has taken place and ACHA should be notified. Send directly to Commissioner and follow up by phone call. D2Commissioner 248.707.4913 -Fall 2010 Eligibility Form Fall Roster must be recieved by Commissioner by October 31st. Participation by any player not meeting minimum academic standards will result in forfeit if team is aware or not. Please review all your players reports prior to playing in any games. Questions contact Commissioner before allowing participation. Recommended to be submitted and reviewed prior to any games. ACHA Men's Division 3 (M3) Documents	Please download, complete and mail the following 2010-11 paperwork: Team Checklist (.rtf) This outlines when information is needed by the ACHA Division 3 Team Application/Declaration (.doc) This is mandatory for ALL teams wishing to play in Division 3 Fall Player Eligibility Form (.xls) This registers players for the fall semester and is due by October 31st. If you play games prior to the deadline and a player is later found to not meet minimum academic standards, your team will face sanctions from game forfeit to removal from the post-season. Winter Player Eligibility Form (.xls) This registers players for the winter semester and is due by January 31st. If you play games prior to the deadline and a player is later found to not meet minimum academic standards, your team will face sanctions from game forfeit to removal from the post-season.
